Booklet order form for intensive care units
Placing an order
The second edition of "Intensive Care - a guide for patients and relatives" will be published in July 2009. Since this 32 page information booklet was first published in 2008, orders have been received from 40% of intensive care units across the country.
If you require more than 500 booklets, please choose 500 booklets in the order form and add a message to say how many booklets you would like at the bottom of the form.
An invoice for the booklets will be sent after they have been delivered to you. If you have a different invoice address, please enter it in the message box at the bottom of the form.
Booklet costs
The cost of printing the current edition of booklets has been paid for from our charity reserves. We need to pass this cost on to recover our reserves.
- 125 booklets - £22.50
- 250 booklets - £45.00
- 375 booklets - £67.50
- 500 booklets - £90.00
Delivery costs
In addition to the cost for the booklet printing, delivery direct from our printers is charged at the following rates.
UK mainland addresses
- 125-250 booklets: £15.00 inc. VAT
- 375-500 booklets: £30.00 inc. VAT
Channel Islands, Isle of Man, Northern Ireland and the Scottish Highlands
- 125-250 booklets: £35.00 inc. VAT
- 375-500 booklets: £60.00 inc. VAT
